How can I transfer my medical records from my previous GP ?

If you wish to transfer your medical records from your previous GP, please print and complete the Transfer of Medical Records form below and bring it with you to your appointment.

Do I need to pay any extra admin fees for the registration and transfer of records

No, you don’t have to pay any additional admin fees to transfer your records from your previous GP to our clinic. You only need to pay the consultation fees.
